You may be eligible to prevent the reporting of adverse information that resulted from human trafficking if you experienced either of the the following:

Sex trafficking: You were recruited, transported, solicited, or harbored for the purpose of a commercial sex act or if you were induced into a commercial sex act by force, fraud, or coercion.

Labor trafficking: You were transported or harbored by force or fraud for slavery, debt bondage, or involuntary servitude.

All of the fields below are required in order to submit a request. The information that you provide will only be used by Plaid Check to verify your identity and to process your request. We may not be able to comply with your request if we are unable to confirm your identity or connect the information you submit in your request with personal information in our possession. If we are unable to process your request, we will notify you via the email you provide.
